Resumen Objetivo: Analizar los resultados y evolución del programa de trasplante hepático del Hospital "Dr. Rafael Ángel Calderón Guardia", así como las complicaciones más frecuentes y características de las hepatopatías que llevaron a trasplante hepático. Métodos: Esta es una investigación retrospectiva que involucra la revisión de expedientes clínicos de los pacientes que recibieron un trasplante de hígado entre los años 2009 y 2018 en el Hospital "Dr. Rafael Ángel Calderón Guardia" en San José, Costa Rica. Se consideraron las siguientes variables categóricas o discontinuas: edad, sexo, nacionalidad, lugar de procedencia, manifestaciones de la hepatopatía, motivo del trasplante, curso clínico postrasplante, comorbilidades, medicamentos empleados, complicaciones, resultados relevantes de exámenes de gabinete y biopsias. Los cálculos estadísticos se llevaron a cabo con paquetes estadísticos STATA, empleando como umbral de significancia estadística un valor de p menor de 0,05. Resultados: La muestra estuvo compuesta de un total de 45 cirugías de trasplante hepático y 44 pacientes que requirieron trasplante de hígado entre abril de 2009 y agosto de 2018, provenientes principalmente de la provincia de San José. El promedio de edad al momento del trasplante para la muestra total fue de 51 años. La hepatopatía que más frecuentemente llevó a trasplante fue la cirrosis etílica, seguida por esteatohepatitis no alcohólica y cirrosis criptogénica. Las complicaciones de la hepatopatía documentadas previo al trasplante: várices esofágicas, sangrado digestivo alto y síndrome hepatorenal. De los pacientes incluidos en el estudio fallecieron 10 en total, lo cual equivale a 22.7%. Conclusiones: La mortalidad observada en los casos de trasplante hepático analizados fue de 22,7%, la mayoría de los casos fueron llevados a trasplante por hepatopatía relacionada con cirrosis etílica, esteatohepatitis y cirrosis criptogénica.


Abstract Objective. To analyze the outcomes, most frequent complications and characteristics of the patients enrolled in the Liver Transplant Program from the Hospital "Dr. Rafael Ángel Calderón Guardia". Methods: This is a retrospective investigation that involves the revision of clinical records from the patients that received a liver transplant between the years 2009 and 2018 in the Hospital "Dr. Rafael Ángel Calderón Guardia". The following variables were considered: age, gender, nationality, city of residence, manifestations of the liver disease, reason for the liver transplant, clinical outcomes after transplant, comorbidities, medication received, important laboratory results and biopsies. The data analysis was performed with STATA, using a statistic significance threshold of a p < 0.05. Results: The sample was composed of a total of 45 liver transplant surgeries and 44 patients who received a liver transplant between the years 2009 and 2018. The patients mostly came from the city of San José. The average age at the time of the surgery was 51 years. The most common liver disease that led to transplant was alcoholic cirrhosis, followed by NASH and cryptogenic cirrhosis. The most common complications of the liver disease documented prior to transplant where esophageal varices, gastrointestinal bleeding and hepatic-renal syndrome. 10 of the patients included in the study died, which corresponds to 22.7% of the sample. Conclusions: The mortality observed in the liver transplant cases analyzed was 22.7%, most of the cases were taken to transplantation due to liver disease related to alcoholic cirrhosis, steatohepatitis and cryptogenic cirrhosis.

ABSTRACT BACKGROUND: In Brazil, the Model for End-Stage Liver Disease (MELD) score is used to prioritize patients for deceased donor liver transplantation (DDLT). Patients with hepatocellular carcinoma (HCC) receive standardized MELD exception points to account for their cancer risk of mortality, which is not reflected by their MELD score. OBJECTIVE: To compare DDLT rates between patients with and without HCC in Rio Grande do Sul, the Southernmost state of Brazil. METHODS - We retrospectively studied 825 patients on the liver-transplant waiting list from January 1, 2007, to December 31, 2016, in a transplant center located in Porto Alegre, the capital of Rio Grande do Sul, to compare DDLT rates between those with and without HCC. The time-varying hazard of waiting list/DDLT was estimated, reporting the subhazard ratio (SHR) of waiting list/DDLT/dropout with 95% confidence intervals (CI). The final competing risk model was adjusted for age, MELD score, exception points, and ABO group. RESULTS: Patients with HCC underwent a transplant almost three times faster than patients with a calculated MELD score (SHR 2.64; 95% CI 2.10-3.31; P<0.001). The DDLT rate per 100 person-months was 11.86 for HCC patients vs 3.38 for non-HCC patients. The median time on the waiting list was 5.6 months for patients with HCC and 25 months for patients without HCC. CONCLUSION: Our results demonstrated that, in our center, patients on the waiting list with HCC have a clear advantage over candidates listed with a calculated MELD score.


RESUMO CONTEXTO: No Brasil, o escore MELD (Model for End-Stage Liver Disease) é utilizado para priorizar os pacientes para transplante hepático de doador falecido (THDF). Pacientes com carcinoma hepatocelular (CHC) recebem pontos de exceção padronizados pelo MELD para contrapesar o risco de mortalidade do seu câncer, que não é refletido pelo seu escore MELD. OBJETIVO: Comparar as taxas de THDF entre pacientes com e sem CHC no Rio Grande do Sul, o Estado mais ao sul do Brasil. MÉTODOS: Foram estudados retrospectivamente 825 pacientes em lista de espera de transplante de fígado entre 1 de janeiro de 2007 e 31 de dezembro de 2016 em um centro de transplantes localizado em Porto Alegre, capital do Rio Grande do Sul, para comparação das taxas de THDF entre aqueles com e sem CHC. Foi estimado o risco variável no tempo de lista de espera/THDF, com relato da taxa de sub-risco (SHR) de lista de espera/THDF/desistência com intervalos de confiança (IC) de 95%. O modelo final de risco competitivo foi ajustado para idade, escore MELD, pontos de exceção e grupo ABO. RESULTADOS: Os candidatos com CHC foram submetidos a um transplante quase três vezes mais rápido do que os pacientes com um escore MELD calculado (SHR 2,64; IC 95% 2,10-3,31; P<0,001). A taxa de THDF por 100 pessoas-mês foi de 11,86 para os pacientes com CHC vs 3,38 para os pacientes sem CHC. O tempo mediano de permanência em lista de espera foi de 5,6 meses para os pacientes com CHC e 25 meses para os pacientes sem CHC. CONCLUSÃO: Nossos resultados demonstraram que, em nosso centro, pacientes em lista de espera com CHC têm evidente vantagem sobre candidatos listados com um escore MELD calculado.

ABSTRACT BACKGROUND: Over the next 20 years, the number of patients on the waiting list for liver transplantation (LTx) is expected to increase by 23%, while pre-LTx costs should raise by 83%. OBJECTIVE: To evaluate direct medical costs of the pre-LTx period from the perspective of a tertiary care center. METHODS: The study included 104 adult patients wait-listed for deceased donor LTx between October 2012 and May 2016 whose treatment was fully provided at the study transplant center. Clinical and economic data were obtained from electronic medical records and from a hospital management software. Outcomes of interest and costs of patients on the waiting list were compared through the Kruskal-Wallis test. A generalized linear model with logit link function was used for multivariate analysis. P-values <0.05 were considered statistically significant. RESULTS: The costs of patients who underwent LTx ($8,879.83; 95% CI 6,735.24-11,707.27; P<0.001) or who died while waiting ($6,464.73; 95% CI 3,845.75-10,867.28; P=0.04) were higher than those of patients who were excluded from the list for any reason except death ($4,647.78; 95% CI 2,469.35-8,748.04; P=0.254) or those who remained on the waiting list at the end of follow-up. CONCLUSION: Although protocols of inclusion on the waiting list vary among transplant centers, similar approaches exist and common problems should be addressed. The results of this study may help centers with similar socioeconomic realities adjust their transplant policies.


RESUMO CONTEXTO: Nos próximos 20 anos, estima-se um aumento de 23% no número de pacientes em lista de espera para transplante de fígado (TxF) e de 83% nos custos no período pré-TxF. OBJETIVO: Avaliar os custos médicos diretos do período pré-TxF sob a perspectiva de um centro de atenção terciária. MÉTODOS: Foram incluídos no estudo 104 adultos em lista de espera para TxF, com doador falecido, entre outubro de 2012 e maio de 2016, tratados integralmente no centro transplantador do estudo. Dados clínicos e econômicos foram obtidos do prontuário eletrônico e do software de gestão hospitalar. Os desfechos de interesse e os custos dos pacientes em lista de espera foram comparados através do teste de Kruskal-Wallis. Um modelo linear generalizado com função de ligação logarítmica foi utilizado para a análise multivariável. Valores de P<0.05 foram considerados estatisticamente significativos. RESULTADOS: Os custos com pacientes submetidos a TxF (US$ 8.879,83; IC 95% 6.735,24-11.707,27; P<0,001) ou que morreram enquanto estavam em lista (US$ 6.464,73; IC 95% 3.845,75-10.867,28; P=0,04) foram maiores do que com pacientes excluídos da lista por qualquer motivo, exceto óbito (US$ 4.647,78; IC 95% 2.469,35-8.748,04; P=0,254) ou daqueles que permaneceram em lista de espera ao final do seguimento. CONCLUSÃO: Embora os protocolos de inclusão em lista de espera variem entre os centros transplantadores, existem condutas semelhantes e problemas comuns devem ser considerados. Os resultados deste estudo podem auxiliar os centros com realidades socioeconômicas semelhantes na adequação das suas políticas de transplante.

ABSTRACT Objective To evaluate the oxidative profile, nutritional status and food intake (caloric value; macronutrients; vitamins A, E and C; and zinc), and to correlate oxidative stress with nutritional status in patients who were candidates for liver transplant. Methods This is a cross-sectional, analytical, and descriptive study with 51 candidates for liver transplant. Sociodemographic and clinical data, anthropometric parameters, food consumption, and a 10mL blood sample were collected from each patient. Oxidative stress was analyzed by the thiobarbituric acid method. The consumption of macronutrients, caloric value and micronutrients (zinc, vitamins A, E and C) were qualitatively analyzed, and zinc was also quantitatively analyzed. Results The mean age was 49.17±8.17 years. The highest percentage of malnutrition was according to arm muscle circumference (56.86%), followed by arm circumference (52.94%), triceps skin fold (50.98%), and body mass index (1.96%). The mean malondialdehyde level was 14.80±8.72μM/L, presenting a negative correlation with the body mass index for patients with liver cirrhosis according to IMC-Campillo values (p=0.001; r=-0.430). Low energy, carbohydrate, protein, vitamin A and E consumption were observed in more than 50% of subjects. Conclusion This study showed an association of nutritional status through body mass index for patients with liver cirrhosis according to IMC-Campillo, with oxidative stress in patients with liver cirrhosis on a liver transplant waiting list.


RESUMO Objetivo Avaliar o perfil oxidativo, o estado nutricional e o consumo alimentar (valor calórico; macronutrientes; vitaminas A, E e C; e zinco), e correlacionar o estresse oxidativo com o estado nutricional em pacientes candidatos ao transplante hepático. Métodos Trata-se de estudo transversal, analítico e descritivo com 51 pacientes candidatos ao transplante hepático. Coletaram-se dados sociodemográficos e clínicos, parâmetros antropométricos, consumo alimentar e amostra de 10mL de sangue de cada paciente. O estresse oxidativo foi analisado por método do ácido tiobarbitúrico. O consumo de macronutrientes, do valor calórico e dos micronutrientes (zinco, vitaminas A, E e C) foi analisado qualitativamente, e o zinco também foi analisado quantitativamente. Resultados A média de idade foi de 49,17±8,17 anos. Maior percentual de desnutrição esteve de acordo com circunferência muscular do braço (56,86%), seguida de circunferência do braço (52,94%), dobra cutânea triciptal (50,98%) e índice de massa corporal (1,96%). A média do malondialdeído foi de 14,80±8,72µM/L, apresentando correlação negativa com os valores do índice de massa corporal para pacientes com cirrose hepática segundo IMC-Campillo (p=0,001; r=-0,430). Observou-se baixo consumo de energia, carboidrato, proteína, e vitaminas A e E em mais de 50% dos indivíduos. Conclusão Este estudo mostrou associação do estado nutricional, por meio do índice de massa corporal para pacientes com cirrose hepática segundo IMC-Campillo, com o estresse oxidativo em paciente com cirrose hepática em lista de transplante de fígado.

Objetivo: descrever a distribuição dos transplantes de órgãos sólidos no Brasil, bem como informações da lista de espera (demanda) e origem dos pacientes transplantados, por tipo de órgão e Unidade da Federação, de 2001 a 2017. Métodos: estudo descritivo, com dados das Centrais Estaduais de Transplantes, da Associação Brasileira de Transplantes de Órgãos e do Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH/SUS). Resultados: 153 centros de transplante foram identificados em 2017, apenas 11,8% deles localizados nas regiões Norte e Centro-Oeste; no período em estudo, foram realizados 99.805 transplantes, variando de 3.520 (2001) a 8.669 (2017); as regiões Sul e Sudeste concentraram o maior número de transplantes. Conclusão: existem desigualdades regionais na realização dos transplantes, possivelmente por não haver uniformidade na distribuição dos serviços.

Métodos: estudio descriptivo usando datos extraídos de Centrales Estatales de Trasplantes, Asociación Brasileña de Trasplantes de Órganos y Sistema de Informaciones Hospitalarias del Sistema Único Nacional de Salud (SIH/SUS). Resultados: 153 centros de trasplante identificados en 2017, solamente 11,8% están ubicados en las regiones Norte y Centro-Oeste; se realizaron 99.805 trasplantes, desde 3.520 (2001) hasta 8.669 (2017); Sur y Sudeste concentran el mayor número de trasplantes. Conclusión: hay desigualdad en el acceso a los trasplantes, posiblemente debido a la falta de uniformidad en la distribución de los servicios.

Conclusion: there are inequalities in transplantation access, possibly due to lack of uniformity in service distribution.

ABSTRACT

El carcinoma hepatocelular (CHC) es una neoplasia con incidencia y prevalencia significativa en el mundo. Ésta en gran porcentaje se origina en el hígado cirrótico. El trasplante hepático bajo condiciones especiales, ha demostrado ser efectivo en el tratamiento simultáneo de ambas condiciones. En América latina la información es escasa. Este es un estudio de tipo descriptivo retrospectivo de corte transversal, cuyo universo es conformado por los primeros 200 trasplantes de hígado de nuestra serie (Años 2000-2015). Durante el periodo mencionado 26 (13%) pacientes tuvieron diagnóstico de CHC. De los 26 trasplantados con CHC, 21 (81%) pacientes fueron de sexo masculino. El promedio de edad fue 59,7 años. La etiología cirrótica más frecuente fue NASH con 9 (35%) casos, seguida de VHC y OH con 6 (23%). El promedio de AFP pre trasplante fue de 113,3. El ¨downstaging¨ fue realizado en 4 pacientes. La sobrevida global obtenida es de 90,9% a más de 5 años. La sobrevida libre de recurrencia en nuestra serie es de 95,5% a 5 años.


Hepatocellular carcinoma (HCC) is a neoplasm with significant incidence and prevalence in the world. In large percentage it originates from a cirrhotic liver. Liver transplantation under special conditions is effective in the simultaneous treatment of both conditions. Information is scarce in Latin America. This is a cross-sectional study of the first 200 liver transplants of our series (Years 2000-2015). During this period, 26 (13%) patients had a diagnosis of HCC. Of the 26 transplanted with HCC, 21 (81%) patients were male. The average age was 59.7 years. The most frequent cirrhotic etiology was NASH with nine (35%) cases, followed by HCV and OH with six (23%). The average of AFP pre-transplant was 113.3. Downstaging was performed in four patients. The overall survival obtained is 90.9% over 5 years. The recurrence-free survival in our series is 95.5% at 5 years.

RESUMO Objetivo: Descrever uma coorte de doentes com insuficiência hepática aguda, e analisar os fatores demográficos e clínicos associados à mortalidade. Métodos: Estudo de coorte retrospectivo em que todos os pacientes admitidos por insuficiência hepática aguda foram incluídos no período de 28 de julho de 2012 a 31 de agosto de 2017. Dados clínicos e demográficos foram coletados via Sistema Epimed. Foram mensurados SAPS 3, SOFA e MELD. Estimaram-se as OR e seus IC95%. Foram obtidas as curvas Características de Operação do Receptor para os escores de prognóstico, assim como a curva Kaplan-Meier de sobrevida para o escore com melhor predição de mortalidade. Resultados: A maioria dos 40 doentes era do sexo feminino (77,5%), e a etiologia mais frequente foi hepatite pelo vírus B (n = 13). Apenas 35% dos doentes foram submetidos ao transplante hepático. A mortalidade hospitalar foi de 57,5% (IC95%: 41,5 - 73,5). Dentre os escores investigados, apenas o SOFA se manteve associado ao risco de morte (OR = 1,37; IC95% 1,11 - 1,69; p < 0,001). Após a estratificação do SOFA em < 12 e ≥ 12 pontos, a sobrevida foi maior nos pacientes com SOFA < 12 (Log-rank p < 0,001). Conclusão: SOFA nas primeiras 24 horas foi o maior preditor de desfecho fatal.


ABSTRACT Objective: To describe a cohort of patients with acute liver failure and to analyze the demographic and clinical factors associated with mortality. Methods: Retrospective cohort study in which all patients admitted for acute liver failure from July 28, 2012, to August 31, 2017, were included. Clinical and demographic data were collected using the Epimed System. The SAPS 3, SOFA, and MELD scores were measured. The odds ratios and 95% confidence intervals were estimated. Receiver operating characteristics curves were obtained for the prognostic scores, along with the Kaplan-Meier survival curve for the score best predicting mortality. Results: The majority of the 40 patients were female (77.5%), and the most frequent etiology was hepatitis B (n = 13). Only 35% of the patients underwent liver transplantation. The in-hospital mortality rate was 57.5% (95%CI: 41.5 - 73.5). Among the scores investigated, only SOFA remained associated with risk of death (OR = 1.37; 95%CI 1.11 - 1.69; p < 0.001). After SOFA stratification into < 12 and ≥ 12 points, survival was higher in patients with SOFA <12 (log-rank p < 0.001). Conclusion: SOFA score in the first 24 hours was the best predictor of fatal outcome.

ABSTRACT BACKGROUND: The Amazon region is one of the main endemic areas of hepatitis delta in the world and the only one related to the presence of genotype 3 of the delta virus. OBJECTIVE: To analyze the profile, mortality and survival of cirrhotic patients submitted to liver transplantation for chronic hepatitis delta virus and compare with those transplanted by hepatitis B virus monoinfection. METHODS: Retrospective, observational and descriptive study. From May 2002 to December 2011, 629 liver transplants were performed at the Walter Cantídio University Hospital, of which 29 patients were transplanted due to cirrhosis caused by chronic delta virus infection and 40 by hepatitis B chronic monoinfection. The variables analyzed were: age, sex, MELD score, Child-Pugh score, upper gastrointestinal bleeding and hepatocellular carcinoma occurrence before the transplantation, perioperative platelet count, mortality and survival. RESULTS: The Delta Group was younger and all came from the Brazilian Amazon Region. Group B presented a higher proportion of male patients (92.5%) compared to Group D (58.6%). The occurrence of upper gastrointestinal bleeding before transplantation, MELD score, and Child-Pugh score did not show statistical differences between groups. The occurrence of hepatocellular carcinoma and mortality were higher in the hepatitis B Group. The survival in 4 years was 95% in the Delta Group and 75% in the B Group, with a statistically significant difference (P=0.034). Patients with hepatitis delta presented more evident thrombocytopenia in the pre-transplantation and in the immediate postoperative period. CONCLUSION: The hepatitis by delta virus patients who underwent liver transplantation were predominantly male, coming from the Brazilian Amazon region and with similar liver function to the hepatitis B virus patients. They had a lower incidence of hepatocellular carcinoma, more marked perioperative thrombocytopenia levels and frequent episodes of upper gastrointestinal bleeding. Patients with hepatitis by delta virus had lower mortality and higher survival than patients with hepatitis B virus.


RESUMO CONTEXTO: A região Amazônica é uma das principais áreas endêmicas da hepatite delta no mundo e a única relacionada com a presença do genótipo 3 do vírus delta. OBJETIVO: Analisar o perfil, mortalidade e sobrevida dos pacientes cirróticos submetidos a transplante hepático por hepatite crônica pelo vírus delta e comparar com os transplantados pela monoinfecção do vírus da hepatite B. MÉTODOS: Estudo retrospectivo, observacional e descritivo. Entre maio de 2002 a dezembro de 2011, foram realizados 629 transplantes de fígado no Hospital Universitário Walter Cantídio, dos quais 29 pacientes foram transplantados por cirrose causada pela infecção crônica do vírus delta e 40 pela monoinfecção crônica da hepatite B. As variáveis analisadas foram: origem, idade, sexo, escore de MELD, classificação de Child-Pugh, ocorrência de hemorragia digestiva alta e carcinoma hepatocelular antes do transplante, número de plaquetas perioperatória, mortalidade e sobrevida. RESULTADOS: O Grupo Delta foi mais jovem e todos oriundos da região Amazônica Brasileira. O Grupo B apresentou maior proporção de pacientes do sexo masculino (92,5%) em relação ao Grupo D (58,6%). A ocorrência de hemorragia digestiva alta antes do transplante, escore de MELD e classificação de Child-Pugh não obtiveram diferenças estatísticas entre os grupos. A ocorrência de carcinoma hepatocelular e a mortalidade foram maiores no grupo com hepatite B. A sobrevida em 4 anos foi de 95% no Grupo delta e 75% no Grupo B com diferença estatisticamente significante (P=0,034). Pacientes com hepatite delta, apresentaram mais acentuada plaquetopenia no pré-transplante e no pós-operatório imediato. CONCLUSÃO: Os pacientes com hepatite por vírus delta submetidos ao transplante hepático eram predominantemente homens, vindos da região da Amazônia brasileira e com função hepática semelhante a dos pacientes com vírus da hepatite B. Apresentavam menor incidência de carcinoma hepatocelular, níveis de trombocitopenia perioperatória mais acentuados e episódios frequentes de hemorragia digestiva alta. Os pacientes com hepatite por vírus delta apresentaram menor mortalidade e maior sobrevida que os pacientes com vírus da hepatite B.

ABSTRACT Objective To establish, measure and analyze quality indicators in the evaluation and enrolment process of patients in a liver transplant program. Methods A quantitative and non-experimental research, with data collected from the database of a liver transplant program, from September 2012 to September 2014. Descriptive statistics were used to analyze the quality indicators identified. Results We analyzed 1,311 visits in the transplant outpatient clinic, most males (66.7%), white (65.1%), mean age of 53 (±12.5) years, from the Southeastern Region (91.2%), and from the State of São Paulo (80.8%). The indicators measured were efficiency of patient admission process (54.8%), efficiency of evaluation of transplant indication (39.9%), efficacy of treatment of patients seen in the program (21.8%), and waiting time to inclusion in the waiting list, median of 84 days (36-142). Conclusion The quality indicators analyzed in this study enabled a quantitative view of the process, facilitating management of results and benchmarking with other transplant centers. Continuous monitoring can optimize resource allocation and planning of services in patient's admission process.


RESUMO Objetivo Estabelecer, mensurar e analisar indicadores de qualidade no processo de avaliação e admissão dos pacientes em um programa de transplante hepático. Métodos Pesquisa de abordagem quantitativa não experimental, com dados coletados a partir do banco de dados da gestão de um programa de transplante hepático entre setembro de 2012 e setembro de 2014. Estatística descritiva foi utilizada para analisar os indicadores de qualidade identificados. Resultados Foram analisados 1.311 atendimentos no ambulatório de transplante, a maioria do sexo masculino (66,7%), branca (65,1%), com média de idade de 53 (±12,5) anos, da Região Sudeste (91,2%) e do Estado de São Paulo (80,8%). Os indicadores mensurados foram eficiência do processo de admissão dos pacientes (54,8%), eficiência da avaliação de indicação ao transplante (39,9%), eficácia do tratamento dos pacientes atendidos no programa (21,8%), e tempo de inclusão na lista de espera, com mediana em 84 dias (36-142). Conclusão Os indicadores de qualidade analisados neste estudo possibilitaram uma visão quantitativa do processo, facilitando o gerenciamento de resultados e o benchmarking entre instituições transplantadoras. O monitoramento contínuo pode otimizar a alocação de recursos e o planejamento de serviços relacionados ao processo de admissão do paciente.

ABSTRACT Background: The incidence of anatomic variations of hepatic artery ranges from 20-50% in different series. Variations are especially important in the context of liver orthotopic transplantation, since, besides being an ideal opportunity for surgical anatomical study, their precise identification is crucial to the success of the procedure. Aim: To identify the anatomical variations in the hepatic arterial system in hepatic transplantation. Methods: 479 medical records of transplanted adult patients in the 13-year period were retrospectively analyzed, and collected data on hepatic arterial anatomy of the deceased donor. Results: It was identified normal hepatic arterial anatomy in 416 donors (86.84%). The other 63 patients (13.15%) showed some variation. According to the Michels classification, the most frequently observed abnormalities were: right hepatic artery branch of superior mesenteric artery (Type III, n=27, 5.63%); left hepatic artery branch of the left gastric artery (Type II, n=13, 2.71%); right hepatic artery arising from the superior mesenteric artery associated with the left hepatic artery arising from the left gastric artery (Type IV, n=4, 0.83%). Similarly, in relation to Hiatt classification, the most prevalent changes were: right hepatic accessory artery or substitute of the superior mesenteric artery (Type III, n=28, 6.05%)), followed by liver ancillary left artery or replacement of gastric artery left (Type II, n=16, 3.34. Fourteen donors (2.92%) showed no anatomical abnormalities defined in classifications, the highest frequency being hepatomesenteric trunk identified in five (01.04%). Conclusion: Detailed knowledge of the variations of hepatic arterial anatomy is of utmost importance to surgeons who perform approaches in this area, particularly in liver transplantation, since their identification and proper management are critical to the success of the procedure.


RESUMO Racional: A incidência das variações anatômicas da artéria hepática varia de 20-50% em diferentes casuísticas. Elas são especialmente importantes no contexto do transplante ortotópico hepático, visto que, além de representar oportunidade ideal para seu estudo anatômico cirúrgico, a sua precisa identificação é determinante para o sucesso do procedimento. Objetivo: Identificar as variações anatômicas no sistema arterial hepático em transplantes hepáticos. Método: Foram analisados retrospectivamente, no período de 13 anos, 479 prontuários de pacientes adultos transplantados, sendo coletados dados referentes à anatomia arterial hepática do doador falecido. Resultados: Identificou-se anatomia arterial hepática normal em 416 doadores (86,84%). Os outros 63 indivíduos (13,15%) apresentaram alguma variação. De acordo com a classificação de Michels, as anomalias mais frequentes foram: artéria hepática direita ramo da artéria mesentérica superior (Tipo III, n=27, 5,63%); artéria hepática esquerda ramo da artéria gástrica esquerda (Tipo II, n=13, 2,71%); artéria hepática direita ramo da artéria mesentérica superior associada à artéria hepática esquerda ramo da artéria gástrica esquerda (Tipo IV, n=4, 0,83%). Do mesmo modo, em relação à Classificação de Hiatt, as variações mais prevalentes foram: artéria hepática direita acessória ou substituta da artéria mesentérica superior (Tipo III, n=28, 6,05%), seguida da artéria hepática esquerda acessória ou substituta da artéria gástrica esquerda (Tipo II, n=16, 3,34%). Quatorze pessoas (2,92%) apresentaram alterações anatômicas sem classificação definida, sendo a de maior frequência o tronco hepatomesentérico, identificado em cinco (1,04%). Conclusão: O conhecimento detalhado das variações da anatomia arterial hepática é de grande importância aos cirurgiões que realizam abordagens nessa região, em especial no transplante hepático, visto que sua identificação e correto manejo são fundamentais para o êxito do procedimento.

ABSTRACT

La polineuropatía amiloidótica familiar asociada a transtiretina (PAF-TTR) es una enfermedad hereditaria con distribución geográfica variable. El objetivo de este trabajo fue presentar nuestra experiencia con pacientes con PAF-TTR. Se evaluaron retrospectivamente 9 casos pertenecientes a diferentes familias. Los criterios diagnósticos utilizados se basaron en la combinación de un cuadro clínico compatible, hallazgos histopatológicos y confirmación genética. Cinco casos mostraron la presentación clásica y 4 la variante de inicio tardío. La mutación p.Val30Met en el gen TTR fue hallada en 6 casos y p.Ala36Pro, p.Thr60Ala y p.Tyr114Cys en los 3 los restantes, respectivamente. La edad media de inicio fue 35 años (rango 26-60). El tiempo medio al diagnóstico fue de 4.2 ± 1.5 años. Siete pacientes recibieron diagnóstico erróneo inicial, 3 de la variante clásica y 4 de la tardía. Nuestra serie de pacientes mostró marcada heterogeneidad en la presentación clínica del grupo de PAF-TTR de inicio tardío, en una región no endémica de Sudamérica.


Transthyretin-related familial amyloid polyneuropathy (TTR-FAP) is a hereditary disease with variable geographical distribution. The aim of this study was to present our experience with TTR-FAP patients. We retrospectively analyzed nine cases belonging to different families. Diagnostic criteria were based on the combination of compatible clinical picture, histopathological findings and genetic confirmation. Five cases showed the classic presentation and other 4 the late onset variant. The p.Val30Met mutation in the TTR gene was found in 6 cases and p.Ala36Pro, p.Thr60Ala and p.Tyr114Cys in the remaining 3, respectively. The median age of symptom onset was 35 years (26-60 range). Mean time to diagnosis was 4.2 ± 1.5 years. Our patient series showed the heterogeneity in clinical presentation of TTR-FAP in a non-endemic region of South America.

OBJECTIVE: The aim of this study was to summarize the experience of a tertiary center in treating hepatoblastoma for the last 21 years. PATIENTS AND METHODS: Fifty-eight cases were included. The tumor extent and prognosis were assessed using the PRETEXT system. The following data were analyzed: age at diagnosis, comorbidities, prematurity, treatment modalities, histopathological findings, surgical details and complications, treatment outcomes, chemotherapy schedules, side effects and complications. Treatment outcomes included the occurrence of local or distant recurrence, the duration of survival and the cause of death. The investigation methods were ultrasonography, CT scan, serum alpha-fetoprotein level measurement and needle biopsy. Chemotherapy was then planned, and the resectability of the tumor was reevaluated via another CT scan. RESULTS: The mean numbers of neoadjuvant cycles and postoperative cycles of chemotherapy were 6±2 and 1.5±1.7, respectively. All children except one were submitted for surgical resection, including 50 partial liver resections and 7 liver transplantations. Statistical comparisons demonstrated that long-term survival was associated with the absence of metastasis (p=0.04) and the type of surgery (resection resulted in a better outcome than transplantation) (p=0.009). No associations were found between vascular invasion, incomplete resection, histological subtype, multicentricity and survival. The overall 5-year survival rate of the operated cases was 87.7%. CONCLUSION: In conclusion, the experience of a Brazilian tertiary center in the management of hepatoblastoma in children demonstrates that long survival is associated with the absence of metastasis and the type of surgery. A multidisciplinary treatment involving chemotherapy, surgical resection and liver transplantation (including transplantations using tissue from living donors) led to good outcomes and survival indexes. .

In 1958 Francis Moore described the orthotopic liver transplantation technique in dogs. In 1963, Starzl et al. performed the first liver transplantation. In the first five liver transplantations no patient survived more than 23 days. In 1967, stimulated by Calne who used antilymphocytic serum, Starzl began a successful series of liver transplantation. Until 1977, 200 liver transplantations were performed in the world. In that period, technical problems were overcome. Roy Calne, in 1979, used the first time cyclosporine in two patients who had undergone liver transplantation. In 1989, Starzl et al. reported a series of 1,179 consecutives patients who underwent liver transplantation and reported a survival rate between one and five years of 73% and 64%, respectively. Finally, in 1990, Starzl et al. reported successful use of tacrolimus in patents undergoing liver transplantation and who had rejection despite receiving conventional immunosuppressive treatment. Liver Transplantation Program was initiated at Hospital Israelita Albert Einstein in 1990 and so far over 1,400 transplants have been done. In 2013, 102 deceased donors liver transplantations were performed. The main indications for transplantation were hepatocellular carcinoma (38%), hepatitis C virus (33.3%) and alcohol liver cirrhosis (19.6%). Of these, 36% of patients who underwent transplantation showed biological MELD score > 30. Patient and graft survival in the first year was, 82.4% and 74.8%, respectively. A major challenge in liver transplantation field is the insufficient number of donors compared with the growing demand of transplant candidates. Thus, we emphasize that appropriated donor/receptor selection, allocation and organ preservation topics should contribute to improve the number and outcomes in liver transplantation.


Em 1958, Francis Moore descreveu a técnica do transplante de fígado em cães. Em 1963, Starzl e sua equipe realizaram o primeiro transplante de fígado. Nos primeiros cinco transplante de fígado, nenhum paciente sobreviveu mais que 23 dias. Até 1977, aproximadamente 200 transplante de fígado tinham sido realizados no mundo. Neste período, foi estabelecida a solução de problemas técnicos do transplante de fígado. Calne, em 1979, utilizou, pela primeira vez, a ciclosporina em dois pacientes submetidos ao transplante de fígado. Starzl e seus colaboradores relataram, já em 1989, que a sobrevida de 1.179 pacientes submetidos ao transplante de fígado em 1 e 5 anos foi, respectivamente, de 73 e 64%. Finalmente, em 1990, Starzl relatou o primeiro uso do novo imunossupressor tacrolimo em pacientes de transplante de fígado que apresentavam rejeição mesmo com o tratamento imunossupressor convencional. O transplante de fígado iniciou-se no Hospital Israelita Albert Einstein em 1990 e já foram realizados mais de 1.400 transplantes. Em 2013, foram realizados 102 transplantes de fígado de doadores falecidos. As principais indicações para o transplante foram carcinoma hepatocelular (38%), cirrose hepática secundária ao vírus C (33,3%) e cirrose alcoólica (19,6%). Destes, 36% dos transplantes apresentavam MELD biológico superior a 30. As sobrevidas do paciente e do enxerto no primeiro ano foram, respectivamente, 82,4 e 74,8%. Um dos maiores desafios da área do transplante de fígado é o número insuficiente de doadores para uma demanda crescente de candidatos ao procedimento. Dessa forma, destacamos que tópicos relacionados à seleção de doadores/receptores, alocação e preservação de órgãos devem contribuir para o aumento e a melhora dos resultados do transplante de fígado.

Objectives: Liver transplantation is the treatment of choice for acute and chronic liver failure, for selected cases of tumors, and for conditions resulting from errors in metabolism. This paper reports the experience of a medical center in Latin America. Methods: Were conducted 305 orthotopic liver transplantations on 284 patients between 2004 and 2010. Of these patients, 241 were adults undergoing their first transplantation. Results: The average age of patients was 52 years old, and 62% of the individuals were male. The most common indication was alcoholic cirrhosis. The rate of patient survival after 1 and 5 years was 82 and 72% respectively. The rate of liver graft survival after 1 and 5 years was 78 and 68% respectively. The main cause of death was sepsis. Complications in the hepatic artery were documented for 5% of the patients. Additionally, 14.5% of the patients had complications in the biliary tract. Infections were found in 41% of the individuals. Acute rejection was observed in 30% of the subjects, and chronic rejection in 3%. Conclusion: In conclusion, liver transplantation at our medical center in Colombia offers good mid-term results, with a complication rate similar to that reported by other centers around the world.


Objetivos: El trasplante hepático es el tratamiento de elección para la falla hepática aguda y crónica, casos seleccionados de tumores y enfermedades por errores en el metabolismo. En este documento se reporta la experiencia de un centro en Latinoamérica. Métodos: Entre el 2004 y el 2010 se realizaron 305 trasplantes ortotópicos de hígado en 284 pacientes. De ellos, 241 corresponde a primer trasplante en adultos. Resultados: La edad promedio fue 52 años y el 62% eran hombres. La principal indicación fue cirrosis por alcohol. La supervivencia de los pacientes a 1 y 5 años fue de 82 y 72% respectivamente y del injerto hepático a 1 y 5 años fue de 78 y 68% respectivamente. La principal causa de muerte fue sepsis. Se documentaron complicaciones en la arteria hepática en el 5% y complicaciones en la vía biliar en el 14.5%. Se encontraron infecciones en el 41% de los pacientes. Rechazo agudo se presento en el 30% y rechazo crónico en el 3%. Conclusión: El trasplante hepático en nuestro centro en Colombia ofrece buenos resultados a mediano plazo, con una tasa de complicaciones similar a lo reportado en otros centros del mundo.
